Also the README still had the old way of applying the patch by extracting, so good thing I had that covered already.

That's probably the problem, the CMS uses the SAA1099 with a non-default clock rate and thus all the frequencies differ. In fact if you check the specs for the SAA1099 chip, you will notice that they use a 8 MHz master clock for the chip. A lot of different devices just use what they already happen...

Thanks, no wonder fiddling with the GUI settings did nothing. Setting the soft_renderer=true boosted my FPS to around 30-40 in the windowed mode which looks very smooth. I have both versions and it seems it is the floppy version that I pulled out of my backup archives. There are install batch files that refer to floppy disks.

ScummVM does not say it is CD version either.
