Monkey Island Madness was a completely separate release, it was only bundled with MI3 by LA marketing. It was also available as a part of LA Archives Vol.3. Not sure if it was ever sold separately though, in any case, I've never seen it in a box, just a CD.

Actually, there was a dead end in the original game, caused by a bug. IIRC, if you go to Spain before Syria you will get stuck. Interestingly, this bug got into the GBA version as well - ran into it myself. As I remember, ScummVM fixes the broken script in the runtime so you don't need to worry.

Moby doesn't say if this pack is a CD or floppies though.
Five HD floppies. Later CD release included most of it but IIRC there were some differences.
There were several games that had extra sound/voices/etc packs of this kind -- Ultima VIII comes to mind.

Detection works now but the game immediately produces this error:
(1:1:0x383): local script 2048 is out of bounds (200,456)!

BTW, I've checked, the game is 256-color.
